Anti-alpha-galactosyl antibody (Anti-Gal) is a naturally occurring, highly abundant antibody in humans, apes, and Old World monkeys, typically comprising about 1% of total circulating immunoglobulins [1, 3, 11]. These antibodies specifically recognize the alpha-gal epitope (Gal-alpha-1,3-Gal-beta-1,4-GlcNAc-R), a carbohydrate structure found on the cell surfaces of most mammals but absent in humans due to the evolutionary loss of the alpha-1,3-galactosyltransferase enzyme [3, 4, 15]. Anti-Gal is a primary mediator of hyperacute rejection in xenotransplantation and the central cause of alpha-gal syndrome, a delayed allergic reaction to red meat and certain drugs (e.g., cetuximab) often induced by tick bites [6, 8, 12]. Conversely, anti-Gal is being harnessed for therapeutic purposes; for instance, cancer vaccines like Algenpantucel-L use alpha-gal epitopes to target tumor cells for destruction by the patient's own anti-Gal antibodies [1, 13]. Additionally, alpha-gal-functionalized nanoparticles are under investigation for their ability to accelerate wound healing and tissue regeneration by recruiting pro-regenerative macrophages through localized complement activation [1, 5].
Drugs interact with anti-Gal antibodies by presenting alpha-gal epitopes, which triggers the antibody to initiate complement-mediated lysis, opsonization via Fc receptors, and recruitment of antigen-presenting cells to the target site [1, 13, 17].
