Overview

Autoreactive CD8+ T cells are a specialized population of cytotoxic T lymphocytes that pathologically recognize and destroy healthy host tissues by identifying self-antigens presented on MHC class I molecules (PMID: 32053138). Under normal physiological conditions, these cells are typically eliminated or suppressed through central and peripheral tolerance mechanisms; however, their escape and activation lead to various autoimmune disorders. In diseases such as Type 1 Diabetes, they selectively destroy insulin-producing beta cells, while in Vitiligo, they target melanocytes (PMID: 31185187). Therapeutic strategies aimed at these cells include systemic immunosuppression, monoclonal antibodies targeting T-cell surface markers like CD3, and emerging precision approaches like antigen-specific tolerization. Monitoring these cells often involves the use of peptide-MHC tetramers to track antigen-specific populations in patient blood or tissue (PMID: 9038344). These cells represent a critical focal point for developing targeted immunotherapies that aim to restore self-tolerance without compromising the entire immune system.
