Overview

The bladder glycosaminoglycan (GAG) layer is a specialized coating of highly hydrated polyanionic polysaccharides located on the luminal surface of the uroepithelium. It is composed of molecules such as hyaluronic acid, chondroitin sulfate, heparan sulfate, and dermatan sulfate, which create a hydrophilic barrier between the urine and the bladder wall (Parsons, 1994; PMID: 8114153). This layer serves as a critical defense mechanism by preventing the adherence of bacteria and the penetration of toxic urinary solutes, such as potassium and urea, into the underlying tissues (Hurst, 1994; PMID: 7525139). In pathological states like interstitial cystitis/bladder pain syndrome (IC/BPS), the GAG layer is often deficient or damaged, leading to increased urothelial permeability and subsequent chronic inflammation and pain (Madersbacher et al., 2013; PMID: 23433131). Therapeutic interventions aim to restore this barrier through the administration of exogenous GAG substitutes, such as intravesical hyaluronic acid or chondroitin sulfate, or systemic agents like pentosan polysulfate sodium (FDA Label, 2020). These treatments help to "seal" the urothelium, reducing the exposure of sensory nerves to irritants and alleviating clinical symptoms.
