Overview

Blood-brain barrier (BBB) efflux transporters are a group of membrane-bound proteins, primarily from the ATP-binding cassette (ABC) transporter family, that actively transport a wide range of lipophilic molecules and xenobiotics out of the brain parenchyma and back into the systemic circulation (StatPearls, 2023). The most prominent members include P-glycoprotein (P-gp/ABCB1), Breast Cancer Resistance Protein (BCRP/ABCG2), and Multidrug Resistance-associated Proteins (MRPs/ABCC) (PubMed, PMID: 25613237). These transporters play a critical role in neuroprotection by preventing the entry of potentially toxic substances; however, they also represent a major hurdle in CNS drug delivery by limiting the brain penetration of many therapeutic agents, including chemotherapeutics and antiepileptics (NIH, 2022). In diseases like epilepsy and brain tumors, these transporters are often overexpressed, leading to multidrug resistance (PubMed, PMID: 30107145). Conversely, their dysfunction or downregulation is implicated in neurodegenerative conditions like Alzheimer's disease, where impaired efflux of amyloid-beta contributes to plaque formation (Nature Reviews Neuroscience, 2017). Pharmacological strategies involve using inhibitors to boost drug delivery or inducers to enhance the clearance of metabolic waste (PubMed, PMID: 22403131).

Competitive or non-competitive inhibition of the transporter's binding site or ATP-hydrolysis domain to prevent the efflux of substrate drugs, thereby increasing their concentration in the brain (PubMed, PMID: 22403131).
