Extracellular calcium ions (Ca2+) are essential electrolytes that serve as vital signaling molecules and structural components within the human body (StatPearls, NBK482128). They are indispensable for physiological processes such as blood coagulation, where they act as a necessary cofactor (Factor IV), and for the maintenance of neuromuscular excitability and bone density (NIH, Calcium Fact Sheet). The concentration of free ionized calcium in the blood is strictly maintained within a narrow range through a complex homeostatic system involving the parathyroid glands, kidneys, and skeletal system (PubMed, 29494014). Therapeutically, extracellular calcium is targeted through direct supplementation with calcium salts to treat deficiency states or through the use of chelating agents like citrate and EDTA, which bind the ions to prevent blood clotting or treat hypercalcemia (PubChem, CID 5460341). Abnormal levels of extracellular calcium are associated with significant clinical pathologies, including hyperparathyroidism, malignancy-associated hypercalcemia, and chronic kidney disease-mineral bone disorder (CKD-MBD) (StatPearls, NBK430713).
Drugs targeting extracellular calcium work either by direct supplementation (e.g., calcium gluconate) to restore physiological levels or by chelation (e.g., EDTA, citrate) to reduce the concentration of free ionized calcium, thereby inhibiting calcium-dependent processes like the coagulation cascade (StatPearls, NBK482128; PubChem, CID 5460341).
