Overview

The Cancer-testis antigen (CTA) peptide-HLA-A*24:02 complex is a specialized therapeutic target in oncology, particularly relevant for immunotherapy in East Asian populations where the HLA-A*24:02 allele is highly prevalent (Gonzalez-Galarza et al., 2020). Cancer-testis antigens are proteins normally expressed only in the immune-privileged environment of the testis but are frequently upregulated in various cancers, including lung, esophageal, and gastric malignancies (Simpson et al., 2005). In this target configuration, a specific peptide fragment derived from a CTA, such as NY-ESO-1 or MAGE-A4, is presented on the tumor cell surface by the HLA-A*24:02 molecule (Shirakura et al., 2012). This peptide-MHC complex serves as a precise 'flag' for recognition by engineered T-cell receptors (TCRs) or vaccine-induced T cells. Therapeutic interventions like TBI-1301 and TBI-1601 utilize TCR-engineered T cells to bind this complex, triggering a potent cytotoxic response against the tumor (Takara Bio Inc., 2023). Because CTAs are absent in most healthy tissues, this target offers a high degree of tumor specificity, reducing the likelihood of systemic toxicity. However, clinical development must carefully monitor for potential cross-reactivity with similar peptides in vital organs and manage risks such as cytokine release syndrome (Linette et al., 2013).
