Overview

Nimacimab is a monoclonal antibody targeting the cannabinoid receptor type 1 (CB1 receptor), predominantly expressed in peripheral tissues. As a negative allosteric modulator, nimacimab inhibits CB1 signaling without significant brain exposure, distinguishing it from prior small-molecule CB1 inhibitors associated with neuropsychiatric risks. Preclinical and early clinical data suggest that peripheral CB1 inhibition via nimacimab promotes weight loss, metabolic improvements, and beneficial changes in hormonal and inflammatory biomarkers. It is under investigation for conditions like obesity, diabetic nephropathy, chronic kidney disease, and steatohepatitis, both as monotherapy and in combination with incretin-based drugs. The main therapeutic innovation is its safety profile, addressing a longstanding barrier in CB1-targeted drug development.

Mechanism of action

Negative allosteric modulation of peripheral CB1 receptor signaling. Inverse agonist and antagonist activity (inhibits CB1 signaling in periphery). Does not cross the blood-brain barrier, thus aiming to avoid CNS side effects.
