Caspase-8 is a cysteine protease that initiates the extrinsic apoptosis pathway and regulates necroptosis, pyroptosis, and autophagy. Indirect inhibition of caspase-8 activation involves mechanisms such as cFLIP proteins competing for DISC binding or dominant-negative splice variants, without directly blocking the enzyme's catalytic site. This modulation has therapeutic implications for diseases involving inflammation, cancer, autoimmunity, and infection. Modulating caspase‐8 activity—especially its indirect inhibition—has implications for diseases involving excessive inflammation (e.g., SIRS/sepsis), cancer immunology, autoimmunity, and infectious disease.
