Overview

The CD4+ T-cell receptor (TCR) specific for Respiratory Syncytial Virus (RSV) fusion (F) protein-derived peptides is a critical component of the adaptive immune system's response to RSV infection. These receptors recognize specific viral epitopes presented by Major Histocompatibility Complex (MHC) class II molecules on the surface of antigen-presenting cells (PubMed: 32616597). Upon recognition, the TCR initiates signaling pathways that lead to the activation and differentiation of CD4+ T-cells into effector subsets, such as Th1 cells, which produce essential cytokines like interferon-gamma and interleukin-2 (PubMed: 28416512). This process is fundamental for providing help to B-cells for high-affinity antibody production and for the optimal activation of CD8+ cytotoxic T-cells. Historically, the nature of the CD4+ T-cell response has been a major safety concern, as Th2-biased responses were linked to vaccine-enhanced respiratory disease (ERD) in early clinical trials (PubMed: 30206144). Modern therapeutic interventions, including the pre-fusion F protein vaccines "Arexvy" and "Abrysvo", specifically aim to engage these TCRs to elicit a protective Th1-skewed memory response. Consequently, monitoring the frequency and phenotype of these specific CD4+ T-cells serves as a vital biomarker for vaccine efficacy and safety.
