The cervical vagus nerve (CVN) is a critical component of the autonomic nervous system, serving as the primary bidirectional communication link between the brainstem and the visceral organs (StatPearls, 2023). It is composed of approximately 80% afferent fibers that relay sensory information to the nucleus tractus solitarius and 20% efferent fibers that provide parasympathetic innervation to the heart, lungs, and gastrointestinal tract (Frontiers in Neuroscience, 2020). In the context of modern therapeutics, the CVN is the primary target for Vagus Nerve Stimulation (VNS) devices, which are FDA-approved for the treatment of refractory epilepsy and treatment-resistant depression (PubMed, 2021). Emerging research also highlights its role in the cholinergic anti-inflammatory pathway, where electrical stimulation of the nerve can inhibit systemic inflammation by reducing the production of pro-inflammatory cytokines like TNF-alpha (Nature Reviews Rheumatology, 2016). While it is primarily a target for bioelectronic devices rather than traditional small molecules, its activity can be modulated by pharmacological agents that enhance cholinergic signaling, such as galantamine (Journal of Internal Medicine, 2011). Therapeutic challenges include the potential for off-target effects such as hoarseness or bradycardia due to the activation of adjacent laryngeal or cardiac fibers (NIH, 2022).
