Overview

Coagulation factor II, commonly known as thrombin in its active form (alpha-thrombin), is a vitamin K-dependent serine protease that plays a central role in the blood coagulation cascade [2][15]. It is synthesized in the liver as the zymogen prothrombin and is activated by the prothrombinase complex (Factor Xa and Factor Va) on phospholipid surfaces [8][15]. The primary function of alpha-thrombin is the conversion of soluble fibrinogen into insoluble fibrin, which forms the structural matrix of a blood clot [2][6]. Additionally, it acts as a potent platelet activator through the cleavage of protease-activated receptors (PARs) and amplifies the coagulation process by activating factors V, VIII, and XI [3][7]. Beyond its procoagulant roles, thrombin also participates in anticoagulation by activating protein C when bound to the endothelial receptor thrombomodulin [4][6]. Due to its critical position in the clotting pathway, alpha-thrombin is a major therapeutic target for anticoagulants such as dabigatran, argatroban, and heparin, which are used to prevent and treat thromboembolic diseases like deep vein thrombosis and stroke [9][11]. Topical formulations of thrombin are also used clinically as hemostatic agents to control surgical bleeding [9]. However, pharmacological inhibition of thrombin carries a significant risk of bleeding, necessitating careful clinical monitoring [9].

Mechanism of action

Drugs targeting alpha-thrombin primarily act through direct or indirect inhibition of its proteolytic activity. Direct thrombin inhibitors (DTIs), such as dabigatran and argatroban, bind to the active site or exosites of the enzyme to block its interaction with substrates like fibrinogen [3][9]. Indirect inhibitors, most notably heparin and its derivatives, function by binding to and accelerating the activity of antithrombin III, which subsequently neutralizes thrombin [2][12]. Additionally, vitamin K antagonists like warfarin reduce the plasma levels of functional thrombin by inhibiting the gamma-carboxylation required for the synthesis of its precursor, prothrombin [2][9].
