Overview

Coagulation factor Xa (FXa) and coagulation factor IIa (Thrombin) are critical serine proteases within the blood coagulation cascade, serving as the primary targets for anticoagulant therapy. Factor Xa is responsible for the rate-limiting step of thrombin generation by converting prothrombin into its active form, thrombin (FIIa), as part of the prothrombinase complex (UniProt P00742). Thrombin subsequently catalyzes the conversion of soluble fibrinogen into insoluble fibrin, which stabilizes blood clots, and further amplifies the cascade by activating factors V, VIII, and XI (UniProt P00734). Dysregulation of these factors is a central driver in the pathogenesis of thromboembolic diseases, including venous thromboembolism, pulmonary embolism, and ischemic stroke associated with atrial fibrillation (StatPearls: Physiology, Coagulation Cascade). Pharmacological intervention typically involves indirect inhibitors like heparin, which enhances antithrombin-mediated neutralization of both factors, or direct inhibitors that specifically bind the active sites of FXa or FIIa (PubMed: 28723317).
