Overview

The Collagen type IV alpha 3 chain (COL4A3) mRNA 3'-untranslated region (3'-UTR) is a critical regulatory sequence that controls the stability and translation of the COL4A3 transcript (UniProt Q01955). This region contains specific binding sites for microRNAs, most notably miR-21, which act to post-transcriptionally downregulate the expression of the alpha 3 chain of type IV collagen (Gomez et al., 2015, JCI). This collagen chain is an essential structural component of the glomerular basement membrane (GBM) in the kidney. In diseases such as Alport syndrome and various forms of chronic kidney disease, the pathological overexpression of miR-21 leads to excessive repression of COL4A3 via its 3'-UTR, contributing to basement membrane thinning, fibrosis, and progressive renal failure (Noone et al., 2013, Pediatric Nephrology). Therapeutic interventions, such as the anti-miR Lademirsen (RG-012), target this regulatory axis by preventing miR-21 from binding to the COL4A3 3'-UTR, thereby restoring or stabilizing collagen production (Regulus Therapeutics, 2023). Modulating this region represents a precision medicine approach to treating genetic and fibrotic nephropathies by maintaining renal structural integrity.
