Cow's milk-specific immunoglobulin E (IgE) is a specialized antibody produced by the immune system that recognizes and binds to specific proteins found in bovine milk, primarily casein, alpha-lactalbumin, and beta-lactoglobulin [1]. In individuals with cow's milk allergy, these IgE antibodies are sensitized and bound to high-affinity FcεRI receptors on mast cells and basophils [2]. Monoclonal antibodies like Omalizumab bind to the Cε3 domain of the IgE molecule, which is the site of binding for both high-affinity (FcεRI) and low-affinity (FcεRII) receptors, thereby neutralizing free IgE and downregulating receptor expression on effector cells [3].
