Overview

Cow's milk protein-specific immunoglobulin E (sIgE) is a specialized class of antibodies produced by the immune system that recognizes and binds to specific allergens found in bovine milk, such as casein and whey proteins (Hochwallner et al., 2014). These antibodies are the primary mediators of Type I hypersensitivity reactions in individuals with cow's milk allergy (CMA) (StatPearls, 2023). When milk proteins are ingested, they cross-link sIgE molecules already bound to high-affinity FcεRI receptors on the surface of mast cells and basophils. This cross-linking triggers the immediate release of potent inflammatory mediators, including histamine, leukotrienes, and cytokines, which cause clinical symptoms (NIH, 2023). Symptoms can range from mild cutaneous reactions to severe, systemic anaphylaxis. In the pharmaceutical industry, this molecule is a key target for anti-IgE biologics like omalizumab, which neutralize circulating IgE and prevent it from sensitizing effector cells (Wood et al., 2024). Furthermore, the levels and affinity of these specific antibodies serve as critical biomarkers for diagnosing the severity of the allergy and monitoring the efficacy of oral immunotherapy. Therapeutic strategies also focus on shifting the immune balance from a Th2-mediated IgE response toward a more tolerant IgG4-dominated response.

Mechanism of action

Anti-IgE monoclonal antibodies bind to the Cε3 domain of the IgE molecule, which is the site that interacts with the high-affinity receptor FcεRI. This sequestration prevents IgE from binding to mast cells and basophils, effectively blocking the allergic cascade before it begins (Wood et al., 2024).
