Overview

Citicoline, also known as cytidine 5'-diphosphocholine (CDP-choline), is a naturally occurring endogenous intermediate in the biosynthesis of phosphatidylcholine, a major structural component of cellular membranes [1, 11]. While it is frequently utilized as a pharmacological agent and dietary supplement, it is a small molecule metabolite rather than a traditional protein target such as a receptor or enzyme [6, 13]. Citicoline functions as a neuroprotective agent by providing the essential precursors, choline and cytidine, for membrane repair and the synthesis of the neurotransmitter acetylcholine [4, 16]. It is clinically indicated for the management of various neurological conditions, including ischemic stroke, traumatic brain injury, and cognitive decline associated with neurodegenerative diseases like Alzheimer's and Parkinson's [8, 14]. By stabilizing neuronal membranes and enhancing mitochondrial energy production, citicoline helps preserve cellular integrity and supports cognitive recovery following brain injury [9, 15].

Mechanism of action

Acts as an essential intermediate in the Kennedy pathway for the biosynthesis of structural phospholipids (primarily phosphatidylcholine) in neuronal membranes; serves as a choline donor for acetylcholine synthesis; restores mitochondrial ATPase and Na+/K+ ATPase activity; and inhibits the activation of phospholipase A2 to reduce the release of free fatty acids [1, 6, 11, 15].
