Overview

Delta(9)-tetrahydrocannabinol (THC) is the principal psychoactive constituent of cannabis. In the intestinal tract, THC primarily interacts with cannabinoid receptors, influencing motility, secretion, and barrier function. It acts as a partial agonist at CB1 receptors located on enteric neurons, inhibiting acetylcholine release and slowing motility. THC also modulates serotonin signaling via antagonism at 5HT3 receptors, contributing to antiemetic properties. Metabolism occurs mainly in the liver by cytochrome P450 enzymes, resulting in active and inactive metabolites. THC is relevant for the management of chemotherapy-induced nausea/vomiting and potentially for modulating abnormal GI motility disorders and inflammatory bowel diseases.
