Overview

The DTwP vaccine is a trivalent immunizing agent composed of diphtheria toxoid, tetanus toxoid, and inactivated whole-cell Bordetella pertussis. The diphtheria and tetanus components are formaldehyde-inactivated exotoxins that retain immunogenicity while losing pathogenicity, effectively training the immune system to neutralize toxins produced during infection (StatPearls: Diphtheria, Tetanus, and Pertussis Vaccine). The whole-cell pertussis component contains a complex mixture of bacterial antigens, which also serves an intrinsic adjuvant function, enhancing the overall immune response to the toxoids (WHO: Pertussis vaccines). This vaccine is designed to induce active immunity, characterized by the production of high-titer neutralizing antibodies and the establishment of memory B and T cell populations. Clinically, it is used to prevent diphtheria, tetanus, and pertussis, which are associated with significant morbidity and mortality in pediatric populations (CDC: Diphtheria, Tetanus, and Pertussis Vaccination). Despite its high efficacy, the whole-cell pertussis component is associated with higher rates of local and systemic adverse effects compared to acellular pertussis vaccines. Consequently, while still widely used globally, many high-income countries have transitioned to acellular formulations (DTaP) to improve the safety profile.
