Bendamustine is a bifunctional alkylating agent that forms covalent bonds with DNA bases through its electrophilic alkyl groups, resulting in intra-strand and inter-strand crosslinking. This DNA damage leads to inhibition of DNA replication and transcription, stem cell cycle arrest, and ultimately cell death via apoptosis and/or mitotic catastrophe. Bendamustine’s effects are distinct from other alkylating agents due to persistent DNA double-strand breaks and unique activation of DNA stress responses, contributing to its efficacy in various cancers and sometimes overcoming resistance seen with older agents.
