Overview

Elastin is a fundamental extracellular matrix protein, encoded by the ELN gene, that provides vital elasticity and structural resilience to tissues including the skin, lungs, and large blood vessels (UniProt: P15502). In the dermis, elastin fibers allow the skin to recoil after mechanical stress; however, their synthesis is largely restricted to early development and declines significantly in adulthood, leading to skin laxity and wrinkle formation (PubMed: 31235110). The therapeutic strategy of delivering the ELN gene to skin cells via a non-replicating, non-integrating Herpes Simplex Virus type 1 (HSV-1) vector, such as the candidate KB301, aims to restore these levels through gene augmentation (Krystal Biotech). By transducing dermal fibroblasts, the vector facilitates the production of tropoelastin, which is then cross-linked into functional elastic fibers within the extracellular space. This approach is primarily targeted at treating signs of skin aging and potentially genetic conditions like cutis laxa, where elastin function is compromised (PubMed: 29601936). Clinical evaluation of such therapies focuses on improving skin thickness and elasticity while monitoring for vector-related immunogenicity or localized inflammation.
