Electroneutral sodium bicarbonate cotransporter 1 (NBCn1), encoded by the SLC4A7 gene, is a critical membrane transport protein that facilitates the 1:1 symport of sodium and bicarbonate ions into cells (PMID: 23382378, 35282311). It serves as a master regulator of intracellular pH by mediating net acid extrusion, which is essential for maintaining cellular homeostasis and protecting against metabolic acidification in various tissues, including the brain, heart, and skeletal muscle (PMID: 3605633, 24841133). In the central nervous system, NBCn1 is localized to synapses and influences neuronal excitability and excitotoxicity through its interaction with scaffolding proteins like PSD-95 (PMID: 35282311, 20140520). In oncology, NBCn1 is significantly upregulated in solid tumors such as breast and colorectal cancer, where it enables cancer cells to survive and proliferate within acidic microenvironments by maintaining a favorable internal pH (PMID: 38310114, 35992135). Pharmacological targeting of NBCn1, using small-molecule inhibitors like S0859 or specific monoclonal antibodies, is being explored as a therapeutic strategy to disrupt pH homeostasis in cancer cells and induce apoptosis (PMID: 38310114, 36046611). However, therapeutic development must account for its physiological roles, as genetic deficiency or inhibition is associated with safety concerns such as hearing loss and altered vascular contractility (PMID: 21148438, 31114385).
