Overview

Endometrial stromal cells (ESCs) are specialized mesenchymal cells that form the connective tissue framework of the uterine lining, the endometrium (NIH, 2014). These cells are highly dynamic and responsive to the cyclic fluctuations of ovarian steroid hormones, particularly estrogen and progesterone, which drive their proliferation and differentiation into decidual cells—a process known as decidualization that is essential for embryo implantation (Gene, 2014; NIH, 2022). ESCs also play a critical role in modulating the local immune environment and facilitating tissue remodeling (CZ CELLxGENE, 2023). Dysregulation of ESC function is a hallmark of several gynecological disorders, including endometriosis, where ESCs grow at ectopic sites, and adenomyosis, where they infiltrate the myometrium (MDPI, 2025; IJBS, 2025). In these diseases, ESCs often exhibit progesterone resistance and increased inflammatory signaling (NIH, 2022). While ESCs are a cell type rather than a single molecular target, they are the primary site of action for numerous therapeutic interventions, including progestins, GnRH agonists, and emerging targeted therapies such as TRPV2 inhibitors and senolytics (European Journal of Pharmacology, 2022; MDPI, 2022).
