The Epidermal Growth Factor Receptor (EGFR) is a transmembrane protein and member of the ErbB family of receptor tyrosine kinases that regulates essential cellular processes including growth, proliferation, and survival (Source: UniProt P00533). Activating mutations in the EGFR gene, primarily exon 19 deletions and the L858R point mutation, cause the receptor to remain in a constitutively active state, driving the progression of non-small cell lung cancer (NSCLC) (Source: PubMed PMID: 15118125). These "sensitizing" mutations increase the affinity of the kinase domain for tyrosine kinase inhibitors (TKIs) compared to adenosine triphosphate (ATP), making them effective therapeutic targets (Source: PubMed PMID: 15958668). The specific absence of the T790M mutation is significant because T790M is a secondary "gatekeeper" mutation that sterically hinders the binding of first- and second-generation TKIs, leading to drug resistance (Source: PubMed PMID: 24931476). Consequently, EGFR activating mutants without T790M represent a patient population highly responsive to early-generation inhibitors like gefitinib and erlotinib, as well as later-generation inhibitors like osimertinib (Source: NCI). Therapeutic intervention targeting these mutants effectively suppresses downstream signaling pathways such as MAPK and PI3K/AKT, leading to tumor regression (Source: PubMed PMID: 15118125). However, clinical management is often complicated by the eventual development of resistance mechanisms and side effects like skin toxicity and diarrhea resulting from the inhibition of wild-type EGFR in healthy tissues (Source: StatPearls).
