Overview

The extracellular fluid (ECF), encompassing the interstitial fluid within the extracellular matrix space and tissue water, constitutes the internal environment surrounding all cells (StatPearls: Physiology, Extracellular Fluid, 2023). It functions as the essential medium for the exchange of nutrients, gases, and metabolic waste between the circulatory system and the cellular interior. Although not a specific molecular target such as a protein or enzyme, the ECF acts as a physiological target for osmotic agents (StatPearls: Osmotic Diuretics, 2023). Pharmacological agents like mannitol are utilized to elevate the osmolality of the ECF, thereby inducing a fluid shift from the intracellular or interstitial compartments into the vasculature to alleviate conditions like intracranial or intraocular hypertension. Pathological expansion of this space, known as edema, is a hallmark of various systemic diseases including congestive heart failure and renal insufficiency. Consequently, the management of ECF volume and tonicity is a fundamental aspect of clinical pharmacology and critical care medicine.
