Overview

The Gamma-aminobutyric acid type A (GABAA) receptor benzodiazepine allosteric site is a high-affinity binding pocket located at the interface of the alpha (α1, α2, α3, or α5) and gamma (γ2) subunits of the pentameric GABAA receptor complex [1.1.1, 1.3.3]. As a key regulatory component of the central nervous system's primary inhibitory receptor, this site facilitates the modulation of chloride ion conductance [1.1.1, 1.2.3]. When occupied by benzodiazepine agonists, the site undergoes a conformational change that increases the receptor's affinity for GABA, resulting in an increased frequency of chloride channel opening and subsequent neuronal hyperpolarization [1.2.2, 1.3.2]. This mechanism underlies the potent sedative, anxiolytic, anticonvulsant, and muscle-relaxant properties of drugs targeting this site [1.1.2, 1.2.1]. It is a major therapeutic target for conditions such as generalized anxiety disorder, insomnia, and various seizure disorders [1.1.2, 1.2.4]. However, chronic pharmacological activation of this site is limited by the risk of developing tolerance and physical dependence, as well as safety concerns regarding respiratory depression and cognitive decline [1.1.4, 1.3.3].
