Ganglioside GD2 lactone is a specific chemical variant of the GD2 ganglioside, formed by the internal esterification of its sialic acid residues (Source: PMID: 1848413). This lactonized form is particularly relevant in the acidic environment of tumors, where it can influence the conformational epitopes presented to the immune system. GD2 and its lactone derivatives are highly expressed on the surface of neuroectodermal tumors, such as neuroblastoma and melanoma, while showing very restricted expression in normal tissues (Source: PMID: 24511214). This differential expression makes it an ideal target for immunotherapy, including monoclonal antibodies and therapeutic vaccines. Drugs targeting this molecule, such as Dinutuximab and Naxitamab, work by recruiting immune effector cells to induce antibody-dependent cellular cytotoxicity (ADCC) and complement-mediated lysis of the tumor cells (Source: FDA Label for Unituxin). However, the presence of GD2 on peripheral nerve fibers leads to significant treatment-related side effects, most notably severe neuropathic pain. Research into the lactone form specifically has often focused on enhancing the immunogenicity of GD2-based vaccines to elicit a more robust anti-tumor response (Source: PMID: 8242214).
