Overview

The H19 promoter is a regulatory DNA sequence that controls the expression of the H19 long non-coding RNA, a gene characterized by its oncofetal expression pattern—highly active during embryonic development but largely silenced in most adult tissues (Mizrahi et al., 2009). In many human malignancies, including bladder, ovarian, and pancreatic cancers, the H19 gene is frequently reactivated and overexpressed, making its promoter an ideal candidate for transcriptionally targeted gene therapy (Amit & Hochberg, 2010). This therapeutic strategy, often referred to as "suicide gene therapy," involves engineering a plasmid where the H19 promoter drives the expression of a potent cytotoxin, specifically Diphtheria Toxin A (DTA). When this construct is delivered into H19-overexpressing tumor cells, the promoter initiates the production of DTA, which irreversibly inhibits protein synthesis by targeting eukaryotic elongation factor 2 (eEF-2), leading to rapid cell death (Smaldone & Davies, 2010). The most advanced drug candidate utilizing this mechanism is Inodiftagene vixotigamide (BC-819), which has been evaluated in clinical trials for the treatment of non-muscle invasive bladder cancer (NCT00085605). This approach offers a high degree of selectivity, as the toxic payload is only produced in cells where the H19 promoter is transcriptionally active, thereby sparing healthy adult tissues (Kim et al., 2010).

Mechanism of action

Transcriptional targeting using a plasmid vector (e.g., BC-819) where the H19 promoter drives the expression of the Diphtheria Toxin A (DTA) gene; upon entry into H19-overexpressing tumor cells, the promoter is activated, leading to the synthesis of DTA which ADP-ribosylates eukaryotic elongation factor 2 (eEF-2), halting protein synthesis and inducing apoptosis (Amit & Hochberg, 2010; Mizrahi et al., 2009).
