Overview

The heat shock protein-peptide complex (HSPPC) is a personalized therapeutic entity composed of a heat shock protein (HSP), such as gp96, HSP70, or HSP90, non-covalently bound to a diverse array of peptides derived from a patient's own tumor [1, 6]. These complexes capture the unique "antigenic fingerprint" of the malignancy, including both tumor-associated antigens (TAAs) and tumor-specific antigens (TSAs) [7, 14]. When used as an autologous vaccine, the HSPPC is internalized by professional antigen-presenting cells (APCs) through specific surface receptors, most notably CD91 (LRP1) [2, 10]. Inside the APC, the chaperoned peptides are processed and cross-presented on both MHC class I and class II molecules [12]. This dual presentation activates a broad population of CD8+ cytotoxic T-lymphocytes and CD4+ helper T-lymphocytes, orchestrating a targeted immune attack against the patient's specific cancer cells [10, 15]. Clinical development of this platform, most notably with the drug Vitespen (Oncophage), has targeted various malignancies including renal cell carcinoma and melanoma [6, 13].
