Overview

Hematopoietic stem and progenitor cell (HSPC) genomic DNA is the fundamental molecular target for ex vivo gene therapies and genome editing interventions designed to treat hereditary blood disorders and metabolic diseases. By modifying the DNA within these multipotent, self-renewing cells, therapeutic platforms can ensure the long-term production of genetically corrected progeny across all hematopoietic lineages (Frangoul et al., NEJM 2021). This target is typically accessed by harvesting CD34+ cells from the patient, followed by the application of viral vectors for gene addition or CRISPR-based tools for precise sequence disruption or repair (Kanter et al., NEJM 2022). In the context of hemoglobinopathies, the DNA is targeted to either insert a functional beta-globin gene or to silence repressors of fetal hemoglobin, such as the BCL11A enhancer (FDA, 2023). Because HSPCs reside at the apex of the hematopoietic hierarchy, any genomic alterations are permanent, making the assessment of off-target effects and insertional mutagenesis critical for patient safety (Nienhuis et al., Molecular Therapy 2006). This target represents a paradigm shift in medicine, moving from chronic symptom management to potentially curative single-dose genetic interventions.
