Overview

The HIV-1 reverse transcriptase peptide-MHC complex is a molecular target formed when the viral enzyme reverse transcriptase is processed into short peptides and presented by Major Histocompatibility Complex (MHC) Class I molecules on the surface of infected cells (PubMed: 15507525). This complex serves as the primary ligand for T-cell receptors (TCRs) on CD8+ cytotoxic T lymphocytes, which are essential for the natural immune control of HIV-1 (NIH: HIV/AIDS Research). In the field of immunotherapy, this complex is targeted by TCR-engineered T-cell therapies (TCR-T) and soluble TCR-bispecific molecules designed to eliminate the latent HIV reservoir (Nature Communications, 2017). Reverse transcriptase is an ideal source of epitopes because it is highly conserved and vital for viral replication, although the high mutation rate of HIV-1 can lead to immune escape through epitope variation (Journal of Virology, 2012). Clinical development often focuses on specific HLA-restricted epitopes, such as the HLA-A*02:01-restricted VIYQYMDDL peptide, to ensure high specificity and patient eligibility (Frontiers in Immunology, 2020). Successful targeting of this complex aims to provide a functional cure by enabling the immune system to recognize and destroy cells harboring the virus. This approach is distinct from traditional antiretroviral therapy as it leverages the cellular immune system to achieve long-term viral suppression. However, the diversity of HLA alleles across the human population requires the development of a library of TCRs to treat a broad patient base.

TCR-mediated recognition of the pMHC complex triggers a signaling cascade in the T cell, leading to the release of perforins and granzymes that induce apoptosis in the HIV-infected target cell (PubMed: 28462337).
