The Human herpesvirus 6 (HHV-6) peptide-HLA complex is the primary molecular target presented on the surface of cells infected with HHV-6A or HHV-6B (Zerr, 2006, PubMed). During the viral life cycle, particularly during reactivation in immunocompromised hosts, viral proteins are processed into short peptides and displayed by Human Leukocyte Antigen (HLA) molecules (Pantry & Medveczky, 2017, PubMed). This presentation allows the immune system, specifically CD8+ and CD4+ T-cells, to identify and eliminate infected cells through T-cell receptor (TCR) mediated recognition (Allovir, 2023). HHV-6 reactivation is a significant cause of morbidity in transplant recipients, leading to conditions such as encephalitis, pneumonitis, and graft failure (Hill et al., 2019, Blood). Therapeutic interventions like Posoleucel (ALVR105) utilize virus-specific T-cells (VSTs) that target these pHLA complexes to control viral replication and restore immune surveillance (Allovir, 2023). The specificity of this interaction is crucial for avoiding off-target effects against healthy host tissues that do not present these viral antigens. Monitoring viral load via DNA PCR and assessing HLA compatibility are essential for the clinical application and efficacy monitoring of these targeted therapies.
