Human leukocyte antigen A*03:01 (HLA-A*03:01) is a highly prevalent MHC class I allele responsible for presenting intracellular peptide fragments to the immune system. Its primary biological function involves binding 8-11 amino acid peptides and displaying them on the cell surface for recognition by CD8+ cytotoxic T-lymphocytes, thereby orchestrating the elimination of virus-infected or cancerous cells. In clinical practice, HLA-A*03:01 has emerged as a crucial predictive biomarker; specifically, multiple sclerosis patients carrying this allele exhibit significantly better clinical outcomes when treated with glatiramer acetate compared to those treated with interferon-beta. Conversely, in oncology, the presence of HLA-A*03:01 is associated with diminished overall survival and poorer responses to various immune checkpoint inhibitors (ICIs), regardless of the tumor type. Furthermore, HLA-A*03:01 serves as a primary restriction element for emerging TCR-engineered T cell (TCR-T) therapies targeting public neoantigens, such as KRAS G12V and PIK3CA mutations, making it a pivotal target for personalized immunotherapy.
