Overview

Human parainfluenza viruses (HPIVs) are a group of four distinct single-stranded, negative-sense RNA viruses within the Paramyxoviridae family that serve as major pathogens of the human respiratory tract (StatPearls, 2023). They are primarily responsible for pediatric respiratory illnesses, including croup, bronchiolitis, and pneumonia, and can cause severe disease in immunocompromised adults (NIH, 2022). The viral infection process is mediated by two surface glycoproteins: the hemagglutinin-neuraminidase (HN) protein, which facilitates binding to host sialic acid receptors, and the fusion (F) protein, which enables the viral envelope to merge with the host cell membrane (NCBI, 2021). While there are currently no FDA-approved specific antiviral therapies or vaccines, pharmacological research focuses on inhibiting these entry proteins and the viral RNA-dependent RNA polymerase (L protein) (PubMed, 2020). Investigational treatments such as DAS181, a recombinant sialidase, aim to deplete host cell receptors, while ribavirin is sometimes used off-label in severe cases despite its significant side-effect profile (ClinicalTrials.gov, 2023).
