The interaction between Immunoglobulin E (IgE) and allergens from the fungus Alternaria alternata, particularly the major allergen Alt a 1, is a critical driver of allergic airway diseases (Gabriel et al., 2016; PubMed: 27156138). Alternaria is one of the most important fungal sources of aeroallergens, strongly associated with the development, severity, and persistence of asthma (Bush and Prochnau, 2004; PubMed: 15242831). When Alt a 1 binds to and cross-links IgE molecules already bound to high-affinity receptors (FcεRI) on the surface of mast cells and basophils, it triggers the release of inflammatory mediators like histamine and leukotrienes. This process leads to the clinical symptoms of allergic rhinitis and asthma. Therapeutic strategies targeting this interaction include anti-IgE monoclonal antibodies, such as Omalizumab, which sequester free IgE to prevent receptor binding and subsequent allergen-induced activation (Xolair Prescribing Information, 2023). Additionally, allergen-specific immunotherapy (AIT) aims to induce immune tolerance by shifting the response from Th2 to Th1/Treg and inducing IgG4 antibodies that compete for allergen binding (Pfaar et al., 2014; PubMed: 24909118). Understanding this interaction is vital for managing patients with mold-induced hypersensitivity.
