Overview

The inferior vena cava (IVC) filter is a small, implantable medical device, typically made of metal alloys like nitinol or stainless steel, designed to mechanically trap blood clots originating from deep vein thrombosis in the lower extremities or pelvis, thereby preventing their migration to the pulmonary arteries and reducing the risk of life-threatening pulmonary embolism. Positioned via catheter in the IVC—a large abdominal vein returning deoxygenated blood to the heart—the filter expands like a basket or cone to capture thrombi while permitting blood flow. It is primarily used in patients contraindicated for anticoagulation therapy, such as those with recent surgery, bleeding disorders, or recurrent clots despite blood thinners. Complications often arise from clot accumulation within the filter, leading to thrombosis, IVC occlusion, or chronic venous insufficiency, which can necessitate interventions like thrombolysis with agents such as tissue plasminogen activator (tPA) or mechanical thrombectomy. While effective for short-term protection, permanent filters carry risks of long-term issues, prompting development of retrievable and bioabsorbable versions; treatments targeting trapped clots include catheter-directed thrombolysis, angioplasty, or stenting to restore flow and mitigate post-thrombotic syndrome. No drugs directly target the filter as a biological molecule, as it functions as a physical barrier rather than a receptor or enzyme.
