Overview

Injured myocardium, specifically necrotic myocardial cells, refers to heart muscle tissue that has undergone irreversible cell death due to acute injury—most commonly from ischemia (lack of blood flow and oxygen). This process is central in conditions such as myocardial infarction (heart attack), where a critical loss of perfusion leads to the death of cardiomyocytes. Necrotic cardiomyocytes lose membrane integrity, causing leakage of intracellular components into the extracellular space. This underlies clinical biomarkers for myocardial infarction diagnosis. The predominant form is coagulation necrosis: affected cells become acidophilic with preserved outlines but altered internal structure. Leukocyte infiltration follows as part of an inflammatory response. Necrotic myocardium triggers robust inflammation aimed at clearing dead cells and initiating repair.
