Overview

The limbal epithelial stem cell (LESC) niche is a specialized anatomical microenvironment located at the limbus, the junction between the transparent cornea and the opaque sclera (NCBI PMC4352205). This niche is essential for maintaining the population of stem cells that continuously replenish the corneal epithelium and prevent the encroachment of conjunctival tissue onto the cornea (StatPearls NBK560758). A deficient LESC niche leads to Limbal Stem Cell Deficiency (LSCD), a condition characterized by corneal neovascularization, chronic inflammation, and loss of transparency, often resulting from chemical burns, Stevens-Johnson syndrome, or genetic disorders like aniridia (PubMed 31513765). While not a single molecular target, the niche represents a therapeutic focus for regenerative medicine, where treatments aim to restore the stem cell population and its supporting environment (EMA Holoclar EPAR). Current therapeutic strategies include the transplantation of autologous or allogeneic limbal tissue, as well as ex vivo expanded stem cell therapies such as Holoclar (Nature 10.1038/s41433-018-0033-z). Research also explores pharmacological modulation of signaling pathways within the niche, such as Wnt and Notch signaling, to stimulate endogenous repair (NCBI PMC6335500).
