Overview

Mindfulness-based stress reduction (MBSR) is an evidence-based therapeutic program developed to help individuals manage psychological distress through structured mindfulness meditation practices. It typically involves group classes over several weeks where participants learn techniques such as focused attention on the present moment, nonjudgmental awareness, acceptance skills training, and mindful movement exercises like yoga.[1][7] Research demonstrates that MBSR can reduce subjective experiences of psychological stress as well as objective physiological markers such as cortisol output and pro-inflammatory gene expression.[2][3][4] The primary mechanisms involve improved attentional control over thoughts/emotions and increased acceptance—skills which together decrease maladaptive reactivity to stressful events.[5] While it has demonstrated benefits across various populations—including those with depression, anxiety disorders, chronic pain syndromes—MBSR itself does not represent a single molecular entity or druggable target but rather operates at the level of behavioral neuroscience by modulating neuroendocrine pathways involved in the human response to chronic psychosocial adversity.[6][8]

Mechanism of action

Mindfulness training reduces psychological and physiological markers of stress through: - Decreased bottom-up reactivity to stressful stimuli in the brain and body[4] - Increased top-down self-regulatory capacity over attention and emotion[3][5] - Downregulation of pro-inflammatory gene expression; upregulation of genes related to adaptive immune function in some studies[2] - Reduction in HPA-axis activation leading to lower cortisol secretion after acute stressors[4]

Stress-related disordersDepression and anxiety disordersChronic pain conditionsOther mental health and psychosomatic conditions
05

Safety considerations

Generally considered safe with minimal risk; rare reports suggest possible exacerbation of distressing thoughts/emotions in vulnerable individuals during intensive meditation practices. No major safety concerns identified.
06

Interacting drugs

None directly; MBSR is not a druggable target but rather an intervention that may influence the efficacy of other treatments.
07

Biomarkers

Cortisol levels as an indicator of HPA-axis activity/stress response[4]Pro-inflammatory cytokine expression profiles from blood samples[2]
