ND4 (NADH:ubiquinone oxidoreductase core subunit 4) is a mitochondrially encoded, hydrophobic transmembrane subunit of complex I. This multiprotein complex is the largest of the respiratory chain components and initiates electron transport by transferring electrons from NADH to ubiquinone, which supports proton translocation and ultimately ATP production. ND4 forms part of the membrane arm essential for proper complex I assembly and function. Dysfunction or mutations of ND4 are linked to mitochondrial diseases, particularly Leber hereditary optic neuropathy, and may contribute to other neuromuscular and neurodegenerative conditions. While specific pharmacological direct targeting is rare, research into gene therapy is ongoing for ND4-related disease.
