Overview

Mucin 16 (MUC16), widely recognized by its clinical biomarker name Cancer Antigen 125 (CA125), is a massive transmembrane glycoprotein belonging to the mucin family. It is normally expressed on the surface of epithelial cells lining the pleura, peritoneum, and female reproductive tract, where it functions in cell lubrication and as a protective barrier against pathogens [UniProt Q8WXI7]. In the context of oncology, MUC16 is significantly overexpressed in epithelial ovarian cancers and is often shed into the serum, making it a critical biomarker for diagnosis and monitoring [NIH/NCI]. Beyond its role as a marker, MUC16 contributes to tumor progression by promoting cell-to-cell adhesion and inhibiting the cytotoxic activity of natural killer (NK) cells, thereby aiding in immune evasion [PubMed: 22553355]. Therapeutic strategies, such as the monoclonal antibody oregovomab, utilize CA125 as a target to facilitate the formation of immune complexes that stimulate a patient's own immune system to recognize and attack tumor cells [PubMed: 15150595]. This approach shifts the role of CA125 from a passive marker to an active focal point for immunotherapy.

Mechanism of action

Oregovomab acts as an immunomolecular enhancer; it binds with high affinity to the CA125 antigen to form an immune complex. This complex is processed by the immune system to induce a vaccine-like effect, specifically triggering the production of anti-idiotypic antibodies (Ab2) and activating a T-cell mediated immune response against tumor cells expressing the CA125 protein [PubMed: 15150595, DrugBank DB05451].
