Overview

The tumor-associated PAM4 epitope is a highly specific glycoform of Mucin 5AC (MUC5AC) that is expressed early in the development of pancreatic neoplasia but is absent from normal pancreatic tissue and benign inflammatory conditions like pancreatitis (Gold et al., 2007, PMID: 17908971). This specificity makes it an ideal target for the early detection and therapeutic intervention of pancreatic ductal adenocarcinoma (PDAC). The epitope is recognized by the monoclonal antibody PAM4 (clivatuzumab), which has been developed into various radioimmunotherapy and antibody-drug conjugate formats (Gold et al., 2013, PMID: 23471001). In clinical applications, drugs like 90Y-clivatuzumab tetraxetan utilize the antibody to deliver ionizing radiation directly to MUC5AC-expressing tumor cells, aiming to bypass the systemic toxicity of conventional chemotherapy (Ocean et al., 2012, PMID: 22370334). Beyond therapy, the PAM4 epitope serves as a potent biomarker for screening high-risk patients, as it can be detected in the serum and biopsy samples of individuals with early-stage pancreatic intraepithelial neoplasia (Gold et al., 2010, PMID: 20406981).
