Overview

Multi-characteristic opsin (MCO-010) is a synthetic, non-mammalian opsin engineered for broad spectral sensitivity and robust signal transduction within the retina. The gene encoding this opsin is packaged into an adeno-associated virus (AAV) vector and delivered via intravitreal injection to specifically target and transduce ON-bipolar cells in human patients who have lost most or all photoreceptors due to retinal degenerative diseases such as retinitis pigmentosa. This optogenetic therapy confers light sensitivity to the bipolar cells, effectively bypassing lost photoreceptors and restoring visual function under ambient light conditions. MCO-010’s multimodal design enables it to respond to a wide range of visible wavelengths, which is a key advantage over earlier microbial or vertebrate opsins that are restricted by spectral tuning and bleaching properties. Clinical trials have shown statistically significant improvements in visual acuity in some patients, with a safety profile that includes mostly mild-to-moderate intraocular inflammation managed by steroids. MCO-010 (sonpiretigene isteparvovec) has received FDA Fast Track and orphan drug designations for use in retinitis pigmentosa and Stargardt disease. This therapeutic approach is considered mutation-agnostic, aiming to restore vision regardless of the underlying genetic cause of photoreceptor loss.
