Bismuth compounds exert their antimicrobial effects through two main mechanisms: direct toxicity to bacteria via heavy metal action and protection of mucosal surfaces, thereby preventing bacterial adhesion and colonization. They are especially effective against *H. pylori* infections associated with peptic ulcers and gastritis and are often used in combination with antibiotics and proton pump inhibitors.
Bismuth acts as a heavy metal that is toxic to bacteria. It disrupts bacterial cellular processes by accumulating within cells, altering structural components, interfering with enzyme activities (such as phospholipase, protease, and urease), and inducing oxidative stress. Bismuth salts also prevent bacteria from binding to mucosal cells in the gastrointestinal tract by blocking adhesion sites on epithelial surfaces.
