Overview

Protamine-insulin complexes, primarily known as Neutral Protamine Hagedorn (NPH) insulin or isophane insulin, are intermediate-acting insulin formulations created by complexing insulin with protamine and zinc at a neutral pH (StatPearls, 2023). This crystalline suspension serves as a subcutaneous depot that slowly releases insulin into the systemic circulation, extending its duration of action compared to regular insulin (NIH, 2024). While often discussed in the context of therapeutic targets, these complexes are technically drug delivery systems or formulations rather than endogenous biological targets like receptors or enzymes (PubChem, 2024). The physiological effect of the released insulin is mediated through the insulin receptor, which facilitates glucose uptake in muscle and adipose tissue while inhibiting hepatic glucose production (PubMed, 2022). NPH insulin is a cornerstone in the management of Type 1 and Type 2 diabetes, providing basal insulin levels to maintain glycemic control (StatPearls, 2023). Clinical challenges associated with its use include the risk of nocturnal hypoglycemia and the necessity for thorough resuspension before injection to ensure dose consistency (NIH, 2024). Rare hypersensitivity reactions to the protamine component can occur, particularly in patients previously exposed to protamine through other medical procedures (PubMed, 2021).

Mechanism of action
