Nontypeable Haemophilus influenzae (NTHi) is a Gram-negative, non-encapsulated bacterium that serves as a common commensal within the human nasopharyngeal microbiome. Colonization is a multi-step process involving adherence to the respiratory epithelium via adhesins like Protein E and PilA, followed by the formation of protective biofilms (PubMed: 30634447). While typically asymptomatic, NTHi colonization is a prerequisite for opportunistic infections, including acute otitis media in children and exacerbations of chronic obstructive pulmonary disease (COPD) in adults (PubMed: 28844623). Current therapeutic strategies focus on eradicating NTHi using antibiotics such as amoxicillin-clavulanate or preventing colonization through vaccines targeting conserved surface antigens like Protein D (PubMed: 31133514). Additionally, the use of commensal bacteria as probiotics is being explored to competitively exclude NTHi from the nasal niche. Because this target represents a complex biological process and microbial community rather than a single protein or receptor, it is more accurately characterized as a therapeutic focus or pathological state.
Interventions target NTHi colonization through various mechanisms: antibiotics inhibit bacterial cell wall or protein synthesis; vaccines induce antibodies against surface proteins to prevent adhesion; and probiotics utilize competitive exclusion to prevent NTHi establishment within the nasal microbiome.
