Overview

The ocular surface, comprising the cornea, conjunctiva, and the overlying tear film, serves as the primary interface between the eye and the environment (StatPearls, NBK470411). Non-specific physicochemical interactions at this site involve the use of topical agents that do not bind to specific receptors but instead modify the physical properties of the tear film to treat conditions like dry eye syndrome (Mayo Clinic, 2023). These agents, often referred to as artificial tears or ocular lubricants, work by enhancing lubrication, increasing tear volume, and stabilizing the lipid, aqueous, or mucin layers of the tear film (AAO, 2023). By reducing friction during blinking and preventing desiccation of the corneal epithelium, these treatments alleviate symptoms of irritation and protect the ocular surface from damage (PubMed, PMC6053339). This approach is fundamental in managing keratoconjunctivitis sicca and other forms of ocular surface disease where the natural tear production or quality is compromised (NIH, NEI). Additionally, these interactions help maintain the optical clarity of the cornea, which is essential for proper visual acuity (PubMed, PMC4411301).

Mechanism of action

The mechanism of action involves the physical stabilization of the tear film and the reduction of friction between the eyelid and the ocular surface. Demulcents like carboxymethylcellulose and hydroxypropyl methylcellulose increase the viscosity of the aqueous layer, prolonging the residence time of moisture on the cornea (StatPearls, NBK470411). Emollients such as mineral oil and petrolatum supplement the lipid layer of the tear film to reduce evaporative water loss (AAO, 2023). Additionally, some formulations utilize osmoprotectants like glycerin to protect corneal epithelial cells from the damaging effects of hyperosmolarity (PubMed, PMC4411301).
