Oncofetal antigens (OFAs) represent a diverse group of proteins that are highly expressed during embryonic and fetal development but are typically silenced or expressed at negligible levels in healthy adult tissues (National Cancer Institute, 2023). In many malignancies, the genes encoding these proteins are reactivated, leading to the re-expression of OFAs on the surface of tumor cells or their secretion into the systemic circulation (PubMed: 31510747). This restricted expression pattern makes them ideal candidates for diagnostic biomarkers and therapeutic targets, as they provide a clear distinction between cancerous and normal cells. Major examples include Carcinoembryonic Antigen (CEA), Alpha-fetoprotein (AFP), and Glypican-3 (GPC3), which are utilized clinically to monitor disease burden and guide treatment decisions (StatPearls: Cancer Markers, 2023). Therapeutic strategies targeting these antigens include monoclonal antibodies, antibody-drug conjugates, and adoptive cell therapies like CAR-T cells, which aim to harness the immune system to eliminate tumor cells expressing these developmental markers (Journal of Hematology & Oncology, 2021). Despite their promise, challenges such as potential low-level expression in normal tissues and the heterogeneity of antigen expression within tumors remain significant hurdles in clinical development.
Drugs targeting oncofetal antigens primarily function through immune-mediated mechanisms, including antibody-dependent cellular cytotoxicity (ADCC), complement-dependent cytotoxicity (CDC), and the activation of T-cells via chimeric antigen receptors (CARs) or bispecific T-cell engagers (BiTEs) (PubMed: 33055128). Additionally, some agents act as antibody-drug conjugates (ADCs) to deliver cytotoxic payloads directly to cells expressing the antigen, or as radioimmunotherapeutics for targeted radiation (NIH: Cancer Treatment Reviews, 2022).
