Plasma osmolality is a measure of the solute concentration in plasma, primarily determined by sodium, glucose, and urea. It is a critical determinant of fluid balance and cell volume, influencing water movement between intracellular and extracellular compartments. The body tightly regulates plasma osmolality through osmoreceptors, ADH, and thirst mechanisms. Disruptions in plasma osmolality can lead to serious clinical consequences, including cellular dehydration, cerebral edema, and neurological dysfunction. Drugs influence plasma osmolality by altering electrolyte and water balance. Diuretics increase water and sodium excretion, reducing osmolality. Vasopressin analogs promote water reabsorption, increasing osmolality. Vasopressin antagonists block vasopressin receptors in the kidney, causing water excretion and decreasing osmolality.
