Overview

Peanut allergen-specific immunoglobulin E (IgE) is a specialized class of antibodies produced by the immune system in individuals sensitized to peanut proteins, such as Ara h 1, Ara h 2, and Ara h 3 (UniProt, 2024). These antibodies circulate in the blood and bind to high-affinity FcεRI receptors on the surface of mast cells and basophils (StatPearls, 2023). Upon subsequent exposure to peanut allergens, the allergens cross-link the surface-bound IgE, triggering the rapid release of inflammatory mediators like histamine and leukotrienes (PubMed, 2022). This process leads to the clinical symptoms of peanut allergy, which can range from mild urticaria to life-threatening anaphylaxis (NIH, 2023). In therapeutic contexts, these antibodies are targeted by monoclonal antibodies such as Omalizumab, which sequesters free IgE and prevents it from binding to its receptors, thereby raising the threshold for an allergic reaction (FDA, 2024). Monitoring the levels of these specific antibodies is a cornerstone of diagnosis and is used to assess the efficacy of treatments like oral immunotherapy (Aimmune Therapeutics, 2020).

Mechanism of action

Binding to the Fc region (specifically the Cε3 domain) of circulating IgE to prevent its interaction with high-affinity FcεRI receptors on mast cells and basophils, thereby inhibiting the allergic cascade and down-regulating receptor expression (NCBI, 2023; PubMed, 2022).
