Pelargonium sidoides extract, commonly known by the standardized preparation EPs 7630, is a herbal medicine derived from the roots of the South African geranium (Timmer et al., 2013). It is primarily indicated for the treatment of acute respiratory tract infections, such as acute bronchitis, the common cold, and sinusitis (Cochrane Database Syst Rev, 2013). The extract is a complex mixture of phytochemicals, including highly oxygenated coumarins like umckalin, gallic acid, and various proanthocyanidins (Kolodziej, 2011). Its therapeutic utility stems from a combination of immunomodulatory, antiviral, and antibacterial properties. By stimulating macrophage activity and increasing the production of nitric oxide and cytokines like interferon-beta, it enhances the body's innate immune response against pathogens (Frontiers in Pharmacology, 2020). Additionally, it has been shown to increase the ciliary beat frequency of respiratory epithelial cells, thereby facilitating the clearance of mucus and trapped pathogens (Phytomedicine, 2011). While generally considered safe, rare instances of hepatotoxicity and mild gastrointestinal distress have been reported (LiverTox, 2018).
The mechanism of action for Pelargonium sidoides extract involves the modulation of the non-specific immune system, specifically through the activation of macrophages and the induction of nitric oxide and pro-inflammatory cytokines such as TNF-alpha and IL-12 (Kolodziej, 2011). It also exhibits direct antiviral effects by inhibiting the attachment and entry of viruses into host cells and interfering with viral replication (Michaelis et al., 2011). Furthermore, the extract enhances mucociliary clearance by increasing the ciliary beat frequency of the airway epithelium, which helps in the mechanical removal of pathogens (Neugebauer et al., 2005).
